Changes to lpt driver:

- ansi prototypes in lpt.c
- a bit of tidying in lpt.c
- ioctl in lpt.c for switching between polling and using interrupts
- added lpt.h - needed for ioctl to allow switching between polling
        and interrupt-driven modes.
